scheduled. You will meet personally with Dr. Epstein in an informal yet
confidential setting. An accurate assessment of your present and future hair
loss pattern will be provided, and a course of treatment decided upon. Helpful

History Questionnaire Package. When completed and mailed back to the Foundation
for Hair Restoration you will be contacted by the office, and have the
